A: The provincial government is now responsible for preparing and sending you a replacement birth certificate. If you lost your birth certificate or if you were born before the 1970's when the churches provided the birth or baptismal certificates, then you will likely need to order an official certificate from the government. This is a requirement if you want to apply for a passport in Canada. I just had to do this, and waited a few months for the forms to arrive. (Now I just have to get the paperwork done for my passport!)
